Minimum Requirements:
• All family members must agree to become a CDH/ADH provider
• Must be 25 years or older
• Legal residency in Arizona/US
• Own or rent a single family detached home
• Are of honest and reputable character
• Provide Personal References
• Able to provide a caring, nurturing and positive home environment
• All household members must be fingerprinted for a criminal history background check and to a background check through Child/Adult Protective Services
• Provide a Physician Statement ensuring physical, emotional, mental stability and ability to provide support and services to children/adults with developmental disabilities
• Submit children 's immunization records
• Show proof of sufficient income to meet the needs of your existing family
• Have reliable transportation
• All pools and spas must be appropriately secured
• All home members must participate in the interview process for the home study
• Complete required training
